DOCTOR’S DUTY

The student should come to the doctor with a sick leave form completed by his doctor and with his medical history.

Leaves of absence for the current semester and issued by one of the specialists (orthopedic traumatologist, cardiologist, heart surgeon, neurologist, neurosurgeon, surgeon, sports medicine doctor, psychiatrist and discharge from hospital, pregnancy – leave from a gynecologist) do not require confirmation from a SWFiS doctor and must be presented on SWFiS teachers’ duty.
