How can I earn medical exemption?
First you need to download the SWFiS medical exemption form, then visit your doctor. Completed form should be submitted either to SWFiS teacher or SWFiS physician during their duty hours in SWFiS Office (if the document was not issued by the specialist listed on our website, it has to be confirmed by SWFiS physician).
Where and when should I present the medical exemption form?
You have to bring the completed form to SWFiS Office and present it during the duty hours of:
- SWFiS teachers (room 2.8) if the medical exemption was issued for the current semester by one of the following specialists: orthopaedic traumatologist, cardiologist, cardiac surgeon, neurologist, neurosurgeon, surgeon, sports medicine physician, psychiatrist, oncologist, or in case of post-hospitalisation exemption (hospital discharge) or pregnancy (exemption from gynaecologist).
- SWFiS sports medicine physician (room 2.9), if the medical exemption was issued by GP or specialist not listed on our website, or the exemption certificate was not issued for the current semester. In case of doubt, SWFiS physician may ask students to bring their medical records with them.
Where can I find information about my medical exemption in USOS?
If your medical condition did not allow you to participate in PE classes which you were registered for, and you completed the course with medical exemption, the information will appear in exam report (next to the name of the course) and in ‘decisions’ (USOSweb → student’s section → my studies → decisions).
If you received the medical exemption at the beginning of semester and you did not register for PE course, the information will appear in ‘decisions’ (USOSweb → student’s section → my studies → decisions).The medical exemption will not appear in the exam report, but it is an equivalent of earning credit for PE course.
Is medical exemption equal to earning credit?
Having a medical exemption is not equal to earning credit. It means that you are exempted from participation in obligatory PE course.
What should I submit to SWFiS physician?
You must have with you the medical exemption form downloaded from SWFiS website and completed by your doctor. It is also recommended to take your medical record with you.
What should I submit to SWFiS teachers during their duty hours in room 2.8?
You must have with you the medical exemption form downloaded from SWFiS website and completed by one of the specialists (orthopaedic traumatologist, cardiologist, cardiac surgeon, neurologist, neurosurgeon, surgeon, sports medicine physician, psychiatrist, oncologist, or gynaecologist – in case of pregnancy) or a hospital discharge (in case of post-hospitalisation exemption).
Should I register for PE course?
Do not register for PE course if you cannot participate in classes due to medical issues. Present your medical exemption certificate for the current semester in SWFiS Office before or within 30 days after the beginning of semester.
